Founded in 1981 by Dr. Samuel LaValley, Polzin & LaValley Family Dentistry offers complete dental care to families from Menominee, MI, and surrounding communities. From the beginning, our practice has nurtured a reputation for kind, patient-focused services. We are passionate about helping our patients achieve their best oral health and partner with them to bring their dream smiles to life.
Our doctors and staff genuinely care about you, your smile, and your family. As a locally owned practice, we believe in building community and giving back to the area we live and work in. Throughout the years, we have worked hard to create an environment where patients from three to 103 years old feel comfortable and cared for. Our practice is newly remodeled to incorporate the latest technology and treatments, including dental implants and laser dentistry. Meet Our Dentists
Dr. Andrea Polzin
Following graduation, Dr. Polzin returned to Menominee to work for her mentor, Dr. Samuel LaValley. She purchased the dental practice from Dr. LaValley in 2009 and has been enjoying the challenge of owning her own business ever since. She and her dental team are very proud to have been voted the best dental office by our local people’s choice award time and time again.
Caring for Every Member of Your Family
Being a mother of three young boys, Dr. Polzin especially enjoys working with children. She has also taken multiple continuing education courses in implant placement and is very proud to have been able to add this service to her patients.
In her free time, Dr. Polzin enjoys spending time with her husband, Bryan and their three boys. She enjoys traveling, camping and going to Packer games. She spends most of her free time watching her boys participate in multiple sports including baseball, basketball, hockey and football.

Dr. Samuel LaValley
Caring for Patients in Menominee for 35+ Years
Dr. LaValley returned to Menominee in 1981 where he opened a private dental practice. He and his wife Judy raised their children here. He is currently working part-time for Dr. Andrea Polzin, who purchased his practice in 2009. From senior patients to young children, he strives to provide each patient with an exceptional dental experience.
Continuing dental education has always been, and will remain an integral part of his practice. He is a member of the American Dental Association, Michigan Dental Association, and the Chicago Dental Society. Dr. LaValley holds a current dental license in Michigan, Wisconsin, and California.
Dr. Andrea Polzin
Dr. Andrea Polzin grew up in the twin cities of Marinette and Menominee. After graduating from Marinette High School, she earned an Associate’s degree from UW-Marinette. From there, she went on to receive a Bachelor of Science degree in Biochemistry from the University of Wisconsin-Madison. In 2003, she began her journey at the Marquette University School of Dentistry and graduated in 2007.
Following graduation, Dr. Polzin returned to Menominee to work for her mentor, Dr. Samuel LaValley. She purchased the dental practice from Dr. LaValley in 2009 and has been enjoying the challenge of owning her own business ever since. In her free time, Dr. Polzin enjoys spending time with her husband, Bryan and their boys Braxton, Dixon, Archer and Griffin. When not at work, you can find her helping her husband design the homes that he builds. She also loves traveling and watching her boys participate in multiple sports including baseball, basketball, hockey, track and football.

Dr. Samuel LaValley
Dr. Samuel LaValley was born and raised in Menominee, MI. After graduating from Marinette Catholic Central High School, he attended Michigan Tech University and received a Bachelor of Science degree in medical technology in 1974. He then obtained his D.D.S. degree from The University of Michigan Dental School in 1978. To gain additional experience, he enlisted in the U.S. Navy where he served as a lieutenant in the Dental Corp for three years.

Meet Our Staff
Caitlin
Dental Hygienist
Caitlin has been a Registered Dental Hygienist since 2013. A devoted wife to Rob and proud mom of Ryder and Reese. On the weekends you can find her cheering on the local hockey team (GO THUNDER!) or relaxing at her family’s cabin in Daggett, MI. As a Milwaukee native, she often makes return trips to visit friends and family. She enjoys
planning Disney World adventures and creating homemade charcuterie boards.
Caitlin says, “When it comes to Oral Health, knowledge is power. ‘When you know better, you do better.’ That’s why I am passionate about leading with education in my approach to help my patients succeed in their oral hygiene goals. I am committed to stay up to date on all of the latest research and technology in order to provide the most comprehensive care possible.
Alicen
Dental Hygienist
Alicen graduated from Madison Area Technical College in May of 2005 with her Associate’s Degree in Dental Hygiene. She loves being a Dental Hygienist and educating her patients with knowledge she has gain through the years. She strives to make her patients’ dental experience as comfortable as possible through her compassion for each individual patient.
Outside of the dental office Alicen likes to spend time with her husband, Eric and their three kids, Eli, Delaney and Bennett. She has two cat that she adores, Oscar and Penelope. The family is very active with sports so you can usually find them at a basketball court, soccer field or a football field!
Kelsay
Dental Hygienist
Kelsay joined out team as a Registered Dental Hygienist in April 2022. She graduated in May 2011 with an Associate’s Degree of Applied Science in Dental Hygiene from NWTC-Green Bay and has been in private practice ever since. In May 2020, she achieved her Bachelor’s Degree of Applied Science in Dental Hygiene from Minnesota State
University, Mankato.
Kelsay is a Veteran of the Army and Air National Guard. She has served on our local fire departments as a volunteer firefighter for 11 years. She now enjoys coaching and watching
her son and daughter play sports.
As a Registered Dental Hygienist, Kelsay strives to deliver thorough and comfortable care by creating a calm and attentive environment for her patients.
Kelsey
Dental Assistant
Kelsey worked as an orthopedic nurse for 6 years prior to joining our team. She started with us in January 2023. She loves working with teeth and working alongside Dr. Polzin.
In her spare time, she enjoys weightlifting and spending time with her two sons, Kash and Kobe. She is a huge foodie and loves all kinds of food.
Tatum
Dental Assistant
Tatum started working at Polzin Family Dentistry during the summer of 2022. She contributes to our office mainly with dental assisting and will also help with front office duties if needed.
Tatum enjoys watching her brothers play sports and spending time with her friends. She loves to travel, explore new hobbies, and binge-watch her favorite show, Bones.
Christine
Front Office Staff
My name is Christine and I’m new to the dental field, starting in June 2023. Before joining the practice, I worked as a CNA for 12 years and a mail carrier for 5 years. I enjoy working at the front desk and interacting with patients and helping them. I also enjoy working with an amazing office family every day.
My husband, Ryan and I reside in Peshtigo. We have three children, Landon, Logan and Alivia and two golden retrievers.
I love traveling and spending time outdoors, especially boating in the summer. Most of all, I love watching my kids at all of their sporting events.
Jessica
Front Office Staff
Jessica has been with Polzin Family Dentistry since 2015 and has 20 years of experience in the dental field. What she enjoys most about her job in the front office is helping patients with their insurance and billing concerns.
Jessica enjoys traveling and spending time at the cottage on the bay with her husband, John and their mini-Bernedoodle, Bernie.
